Medikationsprobleme und Arzneimittelwechselwirkungen verringern<\/h3>\n

[\/vc_column_text][vc_column_text]Besonders \u00e4ltere Menschen nehmen h\u00e4ufig mehrere Medikamente gleichzeitig ein. Dadurch kann es zu arzneimittelbezogenen Problemen, wie z. B. unerw\u00fcnschten Wechsel- und Nebenwirkungen kommen. Folgen k\u00f6nnen zus\u00e4tzliche Krankheitsbilder und Therapiebedarf sein. Relevante medikamentenbezogene Probleme werden oftmals untersch\u00e4tzt und sind h\u00e4ufig die Ursache f\u00fcr Morbidit\u00e4t und Mortalit\u00e4t von Patientinnen und Patienten im Krankenhaus. Im konsortien\u00fcbergreifenden Use Case INTERPOLAR arbeiten die vier Medizininformatik-Konsortien DIFUTURE<\/a>, HiGHmed<\/a>, MIRACUM<\/a> und SMITH<\/a> zusammen, um IT-L\u00f6sungen zur Verbesserung der Arzneimitteltherapiesicherheit zu entwickeln. Riskante Medikation erkennen und gezielt analysieren<\/h4>\n

[\/vc_column_text][vc_empty_space height=“25px“][vc_column_text css=““]Im Use Case INTERPOLAR wird untersucht, wie IT-gest\u00fctzte Verfahren Krankenhausapothekerinnen und -apotheker bei der Identifikation klinisch relevanter und potenziell vermeidbarer Medikationsprobleme unterst\u00fctzen k\u00f6nnen. Das Projekt nutzt hierf\u00fcr die in der Aufbau- und Vernetzungsphase von 2018 bis 2022 im Rahmen der Medizininformatik-Initiative aufgebaute IT-Infrastruktur.[\/vc_column_text][vc_empty_space height=“15px“][vc_column_text css=““]Mit INTERPOLAR wird erstmals eine standort\u00fcbergreifend harmonisierte Routinedokumentation von Medikationsanalysen und der dabei identifizierten medikationsbezogenen Probleme etabliert. Dadurch k\u00f6nnen bislang heterogene Dokumentationspraktiken zwischen Kliniken vereinheitlicht und systematisch multizentrisch ausgewertet werden. Grundlage der IT-gest\u00fctzten Analysen ist eine von Expertinnen und Experten kuratierte Liste klinisch relevanter Kontraindikationen f\u00fcr 700 h\u00e4ufig verordnete Arzneimittel. Eine Kontraindikation bezeichnet einen Faktor (z. B. Alter, Vorerkrankungen, Schwangerschaft, Begleitmedikation), der gegen die Anwendung eines bestimmten Medikaments spricht. Auf Basis der im Projekt entwickelten Kontraindikations-Liste k\u00f6nnen potenzielle Medikationsprobleme algorithmisch identifiziert werden.[\/vc_column_text][vc_empty_space height=“15px“][vc_column_text css=““]Die identifizierten F\u00e4lle werden im Rahmen der INTERPOLAR-Studie retrospektiv durch Stationsapothekerinnen und -apotheker bewertet. Dabei erfolgt sowohl eine patientenbezogene Einsch\u00e4tzung der klinischen Relevanz als auch eine strukturierte Erfassung des Entscheidungs- und Bewertungsprozesses. Ziel ist es zu untersuchen, in welchen Situationen IT-gest\u00fctzte Analysen zus\u00e4tzliche klinisch relevante Medikationsprobleme sichtbar machen und die Arbeit der Stationsapothekerinnen und -apotheker sinnvoll erg\u00e4nzen k\u00f6nnen. Die strukturierte Erfassung dieses klinischen Bewertungsprozesses dient dar\u00fcber hinaus als Grundlage, um Algorithmen zur Unterst\u00fctzung klinischer Entscheidungen weiterzuentwickeln. Insbesondere KI-basierte Methoden wie Large Language Models (LLMs) sollen dadurch verbessert werden, sodass pharmakologisches Expertenwissen systematischer abgebildet und klinische Entscheidungsprozesse k\u00fcnftig besser unterst\u00fctzt werden k\u00f6nnen.[\/vc_column_text][vc_empty_space height=“15px“][vc_column_text css=““]Die INTERPOLAR-Studie wird in mehreren Phasen durchgef\u00fchrt. Zun\u00e4chst werden an den teilnehmenden Datenintegrationszentren die technischen Voraussetzungen f\u00fcr die Zusammenf\u00fchrung, Analyse und Auswertung der Patienten- und Medikationsdaten geschaffen. Anschlie\u00dfend erfolgt eine multizentrische Studie an Universit\u00e4tskliniken mit mehreren beteiligten Stationen und einer gro\u00dfen Kohorte station\u00e4rer Patientinnen und Patienten. Mit Datenanalysen Medikationssicherheit verbessern<\/h4>\n

In POLAR wurden von Februar 2020 bis Dezember 2022 Methoden entwickelt und eingesetzt, um personenbezogene Medikationsdaten aus der Routineversorgung sowie den Arzneimittelverordnungen der Apotheken zu erfassen. Ein ausgew\u00e4hltes Spektrum von Polymedikationen wurde zudem gem\u00e4\u00df verf\u00fcgbarer Methoden hinsichtlich Potenziell Inad\u00e4quater Medikation (PIM) klassifiziert. Dar\u00fcber hinaus wurde ma\u00dfgeblich zur Abbildung der Medikations- und Laborinformationen im MII-Kerndatensatz beigetragen. Das Ziel von POLAR war, das Auftreten von unerw\u00fcnschten Arzneimittelwirkungen und deren Konsequenzen fr\u00fchzeitig zu identifizieren. INTERPOLAR baut auf den Ergebnissen von POLAR auf und wendet diese in der klinischen Praxis an.
